Job Title: Home Health Physical Therapist (PT) 

Position Summary: 

The Home Health Physical Therapist provides skilled physical therapy services to patients in their homes as prescribed by a physician. The therapist evaluates patient needs, develops individualized treatment plans, and delivers interventions that promote mobility, restore function, relieve pain, and prevent disability. The PT works collaboratively with the home health team to support patient recovery and independence. 

 

Key Responsibilities: 

  • Perform comprehensive evaluations of patients’ functional abilities, mobility, balance, strength, and endurance. 

  • Develop and implement individualized treatment plans based on evaluation findings and physician orders. 

  • Provide hands-on therapy, therapeutic exercises, gait and transfer training, and patient/family education. 

  • Instruct patients and caregivers in home exercise programs and safe mobility techniques. 

  • Reassess patients regularly and modify treatment plans as needed to achieve goals. 

  • Document evaluations, daily notes, progress reports, and discharge summaries accurately and timely in compliance with agency, state, and federal guidelines. 

  • Communicate and collaborate effectively with the home health interdisciplinary team, including nurses, occupational therapists, speech therapists, and social workers. 

  • Report patient progress and any changes in condition to the physician and care team. 

  • Participate in case conferences, in-service education, and quality improvement activities. 

  • Ensure compliance with all applicable regulatory, legal, and agency standards. 

 

Qualifications: 

  • Education: Graduate of an accredited Physical Therapy program (Doctor of Physical Therapy (DPT) or equivalent). 

  • Licensure: Current and active Physical Therapy license in the state of practice. 

  • Experience: 

  • Minimum of one year of clinical experience (home health experience preferred). 

  • Familiarity with electronic medical records (EMR) preferred. 

  • Certifications: Current CPR/BLS certification. 

  • Skills: Excellent clinical judgment, interpersonal communication, organization, and time management. 

  • Other Requirements: 

  • Valid driver’s license, reliable transportation, and proof of auto insurance. 

  • Ability to work independently and manage a caseload across multiple home visits. 

 

Physical Requirements: 

  • Ability to lift, move, and transfer patients safely. 

  • Frequent standing, walking, bending, and reaching during therapy sessions. 

  • Regular travel to patient homes within the assigned service area. 

 

Work Environment: 

  • Home-based care setting with varying home conditions. 

  • Interaction with patients, families, and interdisciplinary team members in community settings.
