MHS WORKERS

 

Position Description:

Under the supervision of the clinical director, the MHS Workers will provide direct MHS services that implement a service plan, which provides structure, support, and training and support to enable individuals with significant psychiatric functional limitations to achieve and maintain community stability and independence in the most appropriate, least restrictive environment. MHS may be provided in any setting in which the individual's needs maybe addressed, skills training applied, and recovery experienced.

 

Qualifications: Completion of a Bachelor’s Degree in social work, sociology, psychology, or other DMAS approved human service field and with at least one year of clinical experience providing direct services (developing, conducting, and approving assessments and individual service plans or treatment plans) to persons with a diagnosis of mental illness. A valid Virginia Drivers License is also required for employment. The MHS Workers must meet the requirement for a QMHP-A and fall under one of the following categories:

 

"Qualified Mental Health Professional-Adult (QMHP-A)" means a person in the human services field who is trained and experienced in providing psychiatric or mental health services to individuals who have a mental illness; including:

(i) A doctor of medicine or osteopathy licensed in Virginia;

(ii) A doctor of medicine or osteopathy, specializing in psychiatry and licensed in Virginia;

(iii) An individual with a master's degree in psychology from an accredited college or university with at least one year of clinical experience;

(iv) A social worker: an individual with at least a bachelor's degree in human services or related field (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling or other degree deemed equivalent to those described) from an accredited college and with at least one year of clinical experience providing direct services to individuals with a diagnosis of mental illness;

(v) A person with at least a bachelor's degree from an accredited college in an unrelated field that includes at least 15 semester credits (or equivalent) in a human services field and who has at least three years of clinical experience;

(vi) A Certified Psychiatric Rehabilitation Provider (CPRP) registered with the United States Psychiatric Rehabilitation Association (USPRA); (vii) a registered nurse licensed in Virginia with at least one year of clinical experience; or

(viii) Any other licensed mental health professional.

 

Must be registered with the state and obtained a QMHP license.

 

Duties and Responsibilities:

  1. Conduct intake and orientation sessions with clients
  2. Develop and implement individualized service plans with client and/or parent/guardian in coordination with the Program Clinician and referral agency.
  3. Develop and maintain a network of supportive community referrals for clients
  4. Refer and link clients to community resources
  5. Monitor and ensure the effectiveness of referrals
  6. Conduct monthly and quarterly assessments with clients to evaluate progress and needs.
  7. Establish working relationships with representatives of other agencies who work with and provide services to clients.
  8. Provide care coordination and casework services to clients
  9. Provide training and experiential learning activities to clients as outlined in the client’s individual service plan in daily living skills of menu planning, grocery shopping and meal preparation, job search and maintenance, money management and banking, community resources, health and safety, legal issues, housing search and maintenance.
  10. Provide transportation to clients as needed.
  11. Provide 24 hour on-call coverage to clients on their case load
  12. Maintain, update and review case records
  13. Ensure that all services and casework documentation is submitted in a timely and meets or exceeds licensing, Medicaid standards and Hope Seekers Community Support Services
  14. Report on statistical and utilization data and service outcomes.

P. Coordinates scheduling of appointments, as needed.

Q. Coordinates and provides transportation, as needed.

R. Maintains regular and ongoing contact with referring agencies, as well as other agencies involved in youth and family’s case.

S. Actively coordinates and plans discharge and aftercare plans with referring

Agency.

T. Attends interagency meetings, as necessary.

U. Attends and participates in team meetings.

V. Attends and participates in orientation, in-service training, and conferences as scheduled.

W. Attends supervision.

X. Performs other duties as assigned.

Attend and participate in new employee orientation, in-service training, conferences as scheduled and performs others duties as assigned. Receive 40 hours of training per year, maintain and keep current all first aid and CPR training requirements
